Job summary

The Knolls of Oxford is seeking a Licensed Practical Nurse to join our Skilled Nursing and Assisted Living facility in Oxford, Ohio. You will provide direct patient care, monitor residents' health, administer medications, and communicate changes to the healthcare team.

Qualifications

• Graduate of an accredited nursing program.
• Active LPN license in the State of Ohio.
• Compassionate team player with passion for elderly care.

Responsibilities

• Maintains admission and assessment of new residents.
• Performs rounds and modify nursing procedures as needed.
• Administers medications and documents adverse reactions.
• Manages medication inventories and orders from pharmacy.
• Assists physicians with rounds and contributes to care plans.
• Observes and reports residents' symptoms to physicians and families.
• Transcribes and executes physicians' orders; documents in medical records.
• Monitors nurse-call system and ensures equipment is clean and stocked.
• Uses PPE and adheres to infection-control policies.
• PRN shifts: 3 per month to maintain status.

A Licensed Practical Nurse plays a vital role in our Skilled Nursing and Assisted Living facility by providing direct patient care, monitoring residents’ health, administering medications, and communicating changes in condition to the healthcare team. In This Role You Will:

• Maintains responsibility for admission and assessment of new residents.
• Makes routine rounds of assigned residents and modifies standard nursing procedures for care, as needed.
• Ensures that medications are administered within required time frames and as needed (PRN). Documents any adverse reactions and takes appropriate measures.
• Inventories medication supplies and orders medications from the pharmacy as needed. Reads all medication labels and alerts supervisor of any possible pharmacy or dispensing errors.
• Assists physicians in making rounds, performs nursing procedures, and contributes to residents’ care plan and progress.
• Observes and reports symptoms/conditions of residents to the treating physician, director of nursing, and designated family member(s) in a timely manner.
• Receives, transcribes, and executes physicians’ written or verbal orders.
• Documents pertinent information in residents’ medical records according to departmental policy and applicable State Department of Health regulations.
• Monitors and assists with responding to all nurse call systems and reports malfunctions immediately to the nursing leadership.
• Uses personal protective equipment when providing care and ensures all medical and medication dispensing equipment is clean, sanitized, secured and kept in an orderly manner.
• PRN position: staff are required to do 3 shifts per month to maintain status

Who You Are:

• Graduate of an accredited school of Nursing program
• Maintain an active license in good standing in the State of Ohio
