The 80th Street Residence, an upscale Memory Care community in New York, seeks an experienced LPN Supervisor to oversee clinical care during 12-hour night shifts and support a small team of Home Health Aides in a 70-resident setting. Responsibilities include medication administration, compliance with NYS Department of Health regulations, and ensuring residents receive safe, compassionate care.

Qualifications

• NYS License required
• 2 forms of valid ID required
• PPD test within 30 days or Chest X-ray results
• MMR Test Report
• Social Security and BLS Card
• Must speak/read/write English per NYS DOH regulations

The 80th Street Residence is an upscale Assisted Living Community entirely dedicated to Memory Care for 70 Residents. We are a small community and work very closely together to be sure our Residents receive the best care and socialization possible.

• Your current NYS License
• 2 types of valid ID , such as a Passport, Drivers License, Learner Permit, Non Driver ID card
• PPD test results dated within the last 30 days or copy of Chest X-ray results
• MMR Test Report
• Social Security and BLS Card

Our Nurses work 12-hour shifts and are responsible for medication administration, learning and adhering to NYS Dept. of Health Regulations and the overall well-being of our Residents as well as the oversight of our Home Health Aides As a fully licensed Assisted Living Residence with Certification for both Special Needs and Enhanced Assisted Living, we are required to do pre-employment background checks and drug screening.
