Job summary

Together We Can Staffing Agenc is seeking a Part-Time LPN to join our nursing team for resident care in a facility setting in New York. The role focuses on medication administration, vitals, and collaborative care with RCAs and Memory Care Partners.

Strong documentation and communication are essential for high-quality resident support. Required NYS LPN licensure, a High School Diploma/GED, and Basic First Aid.

Qualifications

• Must have High School Diploma or GED and NYS LPN licensure.
• Basic First Aid Certificate.
• 2 years professional nursing experience; preferred in senior population or Assisted Living.

Responsibilities

• Administers medications to assigned residents according to professional standards, facility policy, and physician’s orders.
• Documents response to medication and medication effectiveness as required.
• Ensures the medication cart is cleaned and stocked daily with paper supplies.
• Ensures medication orders are picked up and medication is ordered from the pharmacy and delivered in a timely manner.
• Performs nursing procedures/treatments to assigned residents according to professional standards, facility policy and physicians orders.
• Documents response and effectiveness of treatments; includes vital sign, specimen, and weight collections.
• Communicates accurate, up-to-date information to Resident Care Assistants and Memory Care Partners; provides direction, feedback, and assistance as needed.
• Produces timely accurate and legally compliant documentation of residents’ status, changes, and observations.
• Educates resident/family members regarding plan of care, medications, procedures and treatments; participates in Family Meetings as necessary.
• Provides continual monitoring of resident’s physical, social, and mental status; reports changes to LPN Manager.
• Maintains and develops work-related knowledge and skills through continuous learning activities.

• 8 Provides continual monitoring of resident’s physical, social, and mental status. Promptly reports changes and concerns to LPN Manager.
• 9 Maintains and develops work-related knowledge and skills through continuous learning activities, attendance at inservices and workshops, etc.
• 10 Completes special projects/tasks in support of effective department functioning as assigned by supervisor.

The following educational minimum requirements must be met by all incumbents:

General

• Must be willing and able to work with older adults in a residential setting with a focus on customer satisfaction.

Education

• High School Diploma/GED; Current NYS LPN licensure; Basic First Aid Certificate

Experience

• 2 years professional nursing experience; previous experience working with senior population/experience in Assisted Living is preferred
