About the Opportunity:

• Start Date: ASAP
• Schedule: Monday to Friday, and e/o Saturday
• Shift: 6:30am-2:30pm
• EMR: SigmaCare

Responsibilities:

• Review referred potential client medical histories submitted by the Intake Department
• Assist with referring clients to appropriate healthcare provider specialists
• Assist with managing the symptoms of withdrawal for clients in the stabilization phase, including referrals for further medical attention
• Conduct health assessments with all new clients
• Assist with managing client prescriptions with various pharmacies
• Administer medications and maintain accurate medication records for review and audit upon request
• Coordinate with primary care physicians to inform treatment decisions
• Implement and follow up on established care plans and monitor progress
• Assist in client referral and follow-up appointments
• Perform other duties as assigned

Qualifications:

• At least 1 year of nursing experience
• High School Diploma / GED
• Graduate of an LPN program
• NY State Licensed Practical Nurse (LPN)
• BLS certification
• Knowledge of substance and alcohol dependence and use populations

Desired Skills:

• Associate's and/or Bachelor's Degree
• Bilingual in English and Spanish
• Methadone treatment experience
• Knowledge of crisis management techniques for potentially violent behavior
