Mosaic Health, LLC. is seeking a Licensed Practical Nurse (LPN) to join our Home Health Team serving Collier County, Florida. You will administer medications, perform wound care, monitor vital signs, and develop individualized care plans under RN supervision.

Requirements include an active Florida LPN license and at least 6 months of clinical experience. Excellent communication and critical thinking are essential, along with reliable transportation to visit patients at home.

We are seeking a Licensed Practical Nurse (LPN) to join our Home Health Team in Collier County. This role provides skilled nursing care under the direct supervision of the Registered Nurse. Performs selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care. Compensation Range

Compensation Range: $24.20 to $36.30 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
