Milford Meadows Care Center is seeking an Evening Shift LPN to provide compassionate, high-quality resident care in a warm, team‑oriented setting. This evening role (2pm–10pm) emphasizes precise medication administration for residents and collaboration with the care team to support well‑being in a homelike environment.

The position requires strong clinical skills, empathy, and the ability to communicate effectively with residents, families, and other professionals.

Responsibilities

• Provide direct care to residents, including administering medications, wound care, and medical treatments as needed
• Conduct vital signs, monitor residents' conditions, and report any changes or concerns to the physicians
• Assist with daily living activities such as bathing, dressing, and grooming, as well as feeding and toileting
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and detailed records of patient care and treatment plans
• Collaborate with other healthcare professionals to ensure seamless delivery of care
• Prioritize and manage competing demands and responsibilities in a fast-paced environment
