Qualifications

• Active New Jersey LPN license.
• 1+ year of nursing experience preferred.
• Current CPR/BLS certification.
• Strong communication and clinical skills.
• Reliable, compassionate, and professional.
• Comfortable working independently in a home care setting.
• Valid driver’s license and reliable transportation preferred.
• EHR/documentation experience preferred.

Responsibilities

• Provide compassionate nursing care based on the client’s individualized care plan.
• Administer medications and prescribed treatments.
• Monitor the client’s condition and communicate changes appropriately.
• Provide wound, catheter, and other skilled nursing care within your scope of practice.
• Maintain accurate and timely documentation.
• Support the client’s comfort, safety, dignity, and independence.
• Communicate professionally with the client, family, and care team.
