The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in the patient's home. Responsibilities include:

1. Providing services within the scope of practice defined by state laws, according to the plan of care, and in coordination with the healthcare team. The plan of care may not be altered.
2. Instructing the patient's family in care and maintaining a healthy environment.
3. Observing, recording, and reporting the patient's response to treatment and any changes in condition to the supervising nurse or physician daily.

License and Certification Requirements

• Current LPN licensure in the state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification required.
