We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team at Kadima Rehabilitation & Nursing at Mt. Lebanon, a state-of-the-art skilled rehabilitation facility located in Mt. Lebanon, Pennsylvania. As an LPN with our team, you will play a vital role in delivering high-quality patient care to individuals recovering from illness, injury, or surgery.

About the Role:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and assisting with daily living activities
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor patient progress, report changes and concerns to the healthcare team, and make adjustments to care plans as needed
• Provide education and support to patients and their families to promote optimal health outcomes
• Participate in quality improvement initiatives to ensure the highest level of patient care and satisfaction
