Kadima Healthcare Rehabilitation and Nursing is seeking an LPN to join our dedicated team in Monroeville, PA. You will provide direct patient care per physician orders and collaborate with RNs and the interdisciplinary team to support residents’ well-being.

Qualifications

• Current licensure as an LPN.
• Active Pennsylvania license (or eligibility).
• Experience in long-term care preferred, but not required.

Responsibilities

• Assess residents’ health status via observation, physical assessment, and review of diagnostic information.
• Administer medications and treatments per physician orders.
• Participate in care planning and collaborate with the interdisciplinary team.
• Monitor, document, and communicate changes in resident condition to physicians and nursing leadership.
• Supervise and support nursing assistants and other care staff.
• Educate residents and families on health conditions and care plans.
• Complete required documentation accurately and in compliance with regulations and facility standards.

Kadima Healthcare Rehabilitation and Nursing is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. In this role, you will provide direct patient care in accordance with physician orders and established care plans while working collaboratively with physicians, RNs, and the interdisciplinary team to support residents’ physical, emotional, and overall well-being.

Key Responsibilities
