Job summary

Twin Oaks Assisted Living in Kansas is seeking a licensed practical nurse (LPN) to join our care team. You will provide patient care per care plans, and collaborate with physicians, patients, and families.

The role reports to the Director of Nursing and involves maintaining clinical records, monitoring compliance with state and federal requirements, and supporting a safe, patient‑centered environment. Competitive pay, benefits, and a scholarship program are offered.

Qualifications

• Currently licensed as an LPN in Kansas.
• Experience in long-term care is a plus.
• Strong communication with patients and families.

Responsibilities

• Provide direct patient care per care plans and coordinate with physicians, patients, families, and staff.
• Communicate patient progress and education to patients and families.
• Review and maintain all clinical records required for patient care and progress.
• Review weekly reports to monitor compliance with state and federal requirements (bloodborne pathogens, hazardous materials, infection control, fire safety).

As an LPN, you will work under the general supervision of the communities Director of Nursing to:

• Provide a provision of care to patients in accordance with their care plans. May require conferences with physician, patient, family members and other staff involved with patient’s care.

• Communicate with patient and family regarding education and progress of patient.

• Review and maintain all clinical records, which are required for patient’s care and progress.

• Review weekly reports to monitor personal and staff compliance to all State and Federal requirements including blood borne pathogens, hazardous materials, infection control, and fire safety
