Job summary

Ohana Pacific Health in Hawaii seeks a Licensed Practical Nurse to deliver resident-centered nursing care at Hale Makua Health Services in Maui. You will apply wound care knowledge, follow established policies, and contribute to a compassionate care environment.

You will administer medications, monitor patient status, and collaborate with physicians and therapy teams to maintain high-quality care for residents.

Qualifications

• Graduate of an accredited nursing program.
• Current Hawaii LPN license.
• Knowledge of medical terminology.
• Experience in geriatrics and current nursing standards preferred.
• TB clearance and health review (two-step) — can be done in-house; First Aid & CPR preferred (in-house).

Responsibilities

• Provide professional nursing care to residents following facility policies and procedures.
• Administer medications and treatments as ordered by the physician.
• Assess patients, noting changes to status and communicating to families and care team.
• Collaborate with physicians, therapy services, consultants and agencies to improve quality of care.

Education

Graduate of an accredited school of nursing.

Current Hawaii LPN license.

The Position:

We're looking for passionate Licensed Practical Nurses to help us with providing professional nursing care to residents following established policies and procedures of the facility.

• Demonstrates knowledge of the patient's diagnoses, limitations, expectations, plan of health, and current status.
• Administers medications and treatments as ordered by the physician and according to policy and procedures.
• Appraises the family, physician and others of changes/incidents.
• Collaborates with physicians, therapy services, consultants and agencies to improve quality of services and to resolve identified problems.

Requirements:

• Graduate of an accredited school of nursing.
• Current license as a License Practical Nurse in the State of Hawaii.
• Knowledge of medical terminology and abbreviation.
• Experience in geriatrics and current nursing standards of practice preferred.
• Current and valid 2 Step TB clearance and health review (can be done in-house).
• First Aid and CPR preferred (can be done in-house).
