Job Description

Following direction of Registered Nurses and/or licensed physicians, provide patient care: perform skilled practical nursing procedures or treatments for clients in assigned work area(s) and/or in the clinic (e.g., insert catheters; reinsert gastrostomy tubes; irrigate ears and wounds; give enemas, douches; collect specimens (urine, stool, sputum); apply sterile dressing, Ace bandages and elastic hose; apply topical ointments.) Assist physicians in examination (e.g., physical exams, treatments, etc.); collect data (e.g., pulse, blood pressure, temperature, weight, measurements, etc.) and assist in emergency (first‑aid, cardiopulmonary resuscitation, restraining clients as required); administer immunizations, controlled medications (e.g. narcotics, tranquilizers) & non‑controlled medications (e.g., aspirin, antibiotics, vitamins) in oral, rectal or injection form (i.e., intramuscular and subcutaneous), provide instructions and information to clients, families and guardians and habilitation staff regarding medication goals, side effects, direct affect, dosage change, etc. Maintain medication accountability records consistent with Medicaid/Medicare, Department, and professional standards, (e.g., Physician’s Order Sheets, Kardex or Interdisciplinary Progress Notes, etc.) Sign utilization record for medications administered from night cabinet. Chart clients record, prepare & count medication utilizing Physician’s Order Sheets, transcribe medication orders to Kardex/ MAR client’s permanent records (Kardex, MAR (medication administration record) or other established document); record medication administered in clients’ charts. Observe and document client’s condition and responses to medication, treatment, behavior and other significant activities. Complete episodic illness plan (OPFR) per procedure. Provide general nursing, direct care and/or programming assistance (e.g., take vital signs; assists with personal grooming and hygiene, bathing, etc., implement self‑administered medication program (e.g., help clients recognize medication, color, instruction on side‑effects, etc.); participate with other program staff in relating to client management and programs, intervention in various client behaviors, such as aggression, agitation, self‑injurious, etc.) Represent health services as a member of the interdisciplinary team, participate in meetings by providing input regarding self‑medications, behaviors, medications, health problems, etc.; complete and present assessments and plans that are reviewed and signed by RN. Collects data for nursing quarterly assessments and annual IPP information as assigned for RN to analyze and sign. Participate in various meetings, (Resident, interdisciplinary, special team meetings, Hab manager meetings, Monthly Nursing meetings, Continuing Education, Mandatory facility meetings). Provide work direction and training to direct care staff in providing health maintenance services of an unskilled nature, (e.g. bathing techniques, positioning, feet elevation, application of lotions, etc.) Prepare oral and written shift reports, interaction with pharmacy regarding medication maintain equipment and supplies. In the absence of an RN at the worksite, may be assigned charge duty with RN and/or physician on call.

Qualifications

License to practice nursing as a licensed practical nurse issued by Ohio Board of Nursing pursuant to Section 4723 Ohio Revised Code; MDS certification issued by ANNAC (i.e. American Association of Nurse Assessment Coordination).

Background check: The final candidate selected for the position will be required to undergo a criminal background check. Rule 5123:2-2-02, “Background Investigations for Employment,” outlines disqualifying offenses that will preclude an applicant from being employed by the Department of Developmental Disabilities.

Hourly wage will be paid at step 1, unless otherwise specified by collective bargaining agreement or rules outlined in the ORC/OAC.
