Job description

Overview

CHC is looking for skilled, compassionate nursing staff to provide around-the-clock expert nursing care to fragile, special needs patients at Children's Habilitation Center. CHC employs registered professional nurses, licensed practical nurses, and certified nursing assistants to care for this unique patient population. CHC nurses have a variety of nursing experiences, including neonatal critical care, pediatric critical care, neurology, orthopedics, cardiac care, tracheostomy wound care, and rehabilitation nursing.

All nursing staff are trained to care for ventilator-dependent, artificial airway patients. The tracheostomy/wound care nurse provides specialized care to our tracheostomy residents and other residents with wound care needs. One certified nursing assistant is assigned to each patient room to provide bedside care and stimulate active treatment. All nursing employees are certified in cardiopulmonary resuscitation. Many professional nursing staff hold certifications in advanced cardiac life support, pediatric advanced life support, pediatric emergency assessment and resuscitation, and intravenous therapy.

The scope of practice for CHC nursing is holistic and involves family participation in patient care whenever possible. We are devoted to nursing care that is safe, compassionate, loving, and stimulating in an atmosphere that is like home.

Responsibilities (example for LPN role)

  • Direct team assistants as needed; wound care with wound care certification.
  • Document the stage, measurements (length, width, depth), and condition (color, odor, drainage) of all decubitus breakdowns in the decubitus care book and medical record; notify the physician of new-onset breakdowns by completing an encounter.
  • Monitor healing processes of breakdowns; notify nursing management and physicians using the Encounter Form.
  • Complete trach tube changes per scheduled trach changes and CHC procedures; document in the medical record, MAR, and charts.
  • Document problems or issues on the interdisciplinary progress notes.
  • Maintain an extra trach and a step-down trach at each patient’s bedside (per doctor’s order).
  • Participate in quality improvement activities; precept new employees and provide training for caregivers/guardians as needed.
  • Ensure all equipment is in working order and supplies are available.
Qualifications
  • Current licensure as a nurse (RN or LPN) or current CNA certification as applicable to the role.
  • CPR certification; many staff hold ACLS, PALS, pediatric emergency assessment and resuscitation, and intravenous therapy certifications.
