Reliant Care Management Company in Missouri is seeking licensed practical nurses to provide compassionate, patient-centered care for residents. We have both full-time and part-time evening shifts available.

You will conduct assessments, administer medications, document changes, and coordinate with CNAs and the medical team. Candidates must hold an active LPN license and demonstrate empathy, integrity, and reliability.

Qualifications

• Active LPN license in Missouri.
• Experience in long-term care or patient care setting preferred.
• Ability to lead CNAs and coordinate care.

Responsabilités

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations

We are currently seeking two LPN’s. We have a full-time and part-time evening shift available.
