Job summary

TALENT Software Services in Flagstaff, AZ is seeking a licensed practical nurse to join our unit. You will administer medications and perform treatments for residents, coordinate with the interdisciplinary team, and support care plans and documentation.

Schedule 6:30pm–6:00am, full-time or PRN. Responsibilities include admissions assessment, ongoing care plan development, QA/QIP participation, shift reports, and clear communication with residents, families, and staff.

Responsibilities

• Administer medication and perform treatments on an assigned unit as ordered by the physician and within the scope of license.
• Observe and report the effectiveness of medications and treatments.
• Ensure an adequate amount of supplies and medication are available.
• Receive and deliver a variety of information regarding the resident's medical, social, functional, and other needs.
• Participate in the receiving and giving shift reports.
• Communicate with residents, family, and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team.
• Complete the initial admission nursing assessment tool on a newly admitted resident.
• Assist with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews, and care plans.
• Maintain appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress.
• Participate in our Quality Assurance and Performance Improvement (QAPI) program.
• Attend and participate in staff meetings.

• Block scheduling which rotates. Always 2 days off in a row and every other weekend.

Responsibilities include:

• Administer medication and perform treatments on an assigned unit as ordered by the physician and within the scope of license. Medications may be in various forms including oral, enteral, parenteral, suppository, etc. Treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care, and catheter care.
• Observe and report the effectiveness of medications and treatments.
• Ensure an adequate amount of supplies and medication are available.
• Receive and deliver a variety of information regarding the resident's medical, social, functional, and other needs.
• Participate in the receiving and giving shift reports.
• Communicate with residents, family, and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team.
