Job summary

Fair Oaks Health & Rehabilitation in Fairfax, VA is seeking compassionate LPNs for full- and part-time Day, Evening and Night shifts. Rate of pay ranges from $31.50 to $36.52 per hour, with shift differentials as applicable.

New grads welcome; CPR/BLS required. Responsibilities include administering medications, providing skilled nursing care, and collaborating with RNs and CNAs to support residents’ physical and emotional well-being.

Qualifications

• Current LPN license in Virginia.
• CPR/BLS certification (or willing to obtain).
• Previous LTC or SNF experience preferred, but new grads welcome.
• Positive attitude, reliability, and strong clinical judgment.

Responsibilities

• Provide nursing care to ensure resident needs are met per standards of practice, orders, policies and guidelines.
• Administer medications and treatments with precision and compassion.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents’ physical, emotional, and psychological well-being.
• Assist with resident comfort and safety.
• Collect samples, monitor catheters, and provide wound care.
• Maintain confidentiality and follow all policies, guidelines and legal requirements.
• Record and review patient vital signs and medical history; monitor changes; assist with care coaching.

Shift Availability

We offer Full-Time and Part-Time Day (7a–3p), Evening (3p–11p), and Night (11p–7a) shifts. Available schedules may vary based on current facility needs.

Rate of pay: $31.50 to $36.52 (plus Shift Diffs as applicable)

What You’ll Do

• Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

• Administer medications and treatments with precision and compassion

• Collaborate with RNs, CNAs, and interdisciplinary teams

• Support residents’ physical, emotional, and psychological well-being

• Assist with resident comfort

• Collect samples and monitor catheters. Provide wound care.

• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.

• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.

• Help coach staff as necessary.

Requirements

• Current LPN license in Virginia

• CPR/BLS certification (or willing to obtain)

Salary/ Wage Range

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
