Compensation: Starting at $35/hour, with flexibility based on experience

Coverage Area: Eugene, OR and surrounding communities

Key Responsibilities

• Deliver direct patient care in accordance with individualized care plans
• Monitor vital signs, administer medications, and report changes in condition
• Provide wound care, catheter care, and other hands-on procedures as needed
• Educate patients and families on treatment plans, health goals, and self-care practices
• Collaborate with RNs, therapists, and the interdisciplinary team to ensure quality and continuity of care
• Maintain timely, accurate documentation of care provided
  
  

Qualifications

• Active LPN license in the state of Oregon
• Minimum of one (1) year of LPN experience (home health experience a plus)
• Current CPR certification
• Valid driver's license and reliable, insured transportation
