Job summary

Conviva Senior Primary Care, part of Humana, is hiring an LPN/LVN to provide nursing care within a designated unit or clinic. You will assess health problems, take vitals, administer medications, develop care plans, and maintain medical records.

This patient-facing role includes TB screening and collaboration with a care team. The ideal candidate holds current LPN/LVN licensure, demonstrates excellent customer service, and has a strong clinical background.

The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region.

The LPN/LVN assesses patient health problems and needs, takes vital signs, administers medication, develops and implements nursing care plans, and maintains medical records. May advise patients on health maintenance and disease prevention or provide case management. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Required Qualifications

• Current licensure as an LPN or LVN
• This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
• Excellent customer service

• Experience in a fast pace/high volume environment

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$48,900 - $66,200 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
