Qualifications

  • Graduate of a National League for Nursing accredited program.
  • Licensed as a Practical Nurse with at least three years of clinical experience.

Premise Health is proud to partner with Cummins Wellness Center and is currently looking for a full-time Licensed Practical Nurse (LPN) to join our team in Columbus, IN!

Schedule
  • 4 - 10 hour shifts per week 7a-7p
  • 3 Days per Week 7a-6p
  • 1 Day per Week 8a-7p
  • Saturday Rotation approximately every 6 to 8 weeks 8a-12p
Essential Functions
  • Interview patients to determine medical problems/conditions and document in charts for physicians.
  • Obtain and record patients' vital signs and weight.
  • Assist physicians with patient examinations as needed.
  • Review physicians' orders, lab requests, or follow-up needs with patients.
  • Document services performed on appropriate forms.
  • Dress wounds as instructed. Apply compresses, ice, and heat packs.
  • Observe patients and report adverse reactions to medication or treatment to medical personnel in charge.
  • Administer specified medication, orally or by injections, and note time and amount on patients' charts.
  • Assemble and use clinic equipment.
  • Collect samples such as urine, blood, and sputum for testing and perform routine laboratory tests on samples.
  • Sterilize equipment and supplies.
  • Clean rooms and change linens.
  • Maintain supplies, including stocking, ordering new supplies, and disposing of expired or damaged supplies.
  • Respond to patient or client telephone inquiries by evaluating and addressing problems and questions.
  • Follow up with patients who miss appointments.
  • Follow up on lab results, including scheduling additional tests if needed.
  • Coordinate referrals for patients through insurance and other physician offices.
  • This job has no supervisory responsibilities.
  • Other duties as assigned.
Job Requirements
  • Graduate of a National League for Nursing accredited program, an Associate's degree (A.S.), or equivalent from a two-year college or technical school.
  • Licensed as a Practical Nurse.
  • Current certification in Basic Life Support is required; Advanced Cardiac Life Support may also be required based on scope of contract services.
  • At least three years of clinical experience as an LPN in ambulatory care, occupational health, or emergency department settings.
