Job summary

Remote Care Partners is seeking a Remote Virtual Nurse Health Coach (LPN) to support patients across multiple states from home. You will educate patients on monitoring equipment, review vital signs, and collaborate with providers to update care plans.

This full-time, remote role requires active LPN licensure, EMR proficiency, and experience with chronic conditions such as diabetes, COPD, asthma, CHF, or atrial fibrillation. You’ll empower patients to follow plans and prevent urgent visits.

Qualifications

• 2 years nursing experience, case management or home health preferred
• Active Compact Licensure required
• EMR experience required
• Computer competency
• Experience with chronic conditions (diabetes, COPD, asthma, CHF, A Fib) preferred

Responsibilities

• Welcomes patients and explains remote monitoring benefits
• Educates patients on equipment used for remote monitoring
• Reviews vital signs in near real-time and escalates when needed
• Obtains actionable information from patients with abnormal readings
• Uses EMR to coordinate with providers for interventions
• Assists providers in creating patient-centered care plans
• Monitors adherence to plans and device usage
• Coaches patients to adopt healthier behaviors
• Remains first point of contact for motivational factors

Virtual Nurse Coach - LPN - Remote location! Positions available in all 50 states!!!

The Role: Remote Virtual Nurse Health Coach

Key Job Functions:

• Welcoming patients to benefits of remote monitoring
• Educating patients on equipment they will use
• Near real time review of patient vital signs
• Obtaining actionable information from patients with abnormal readings
• Using practice EMR to escalate patient situations to the provider for medical intervention
• Reviewing & assisting the provider in the creation of a patient centered care plan
• Monitoring notifications and observing trends
• Creating patient centered care plans as well as reviewing monthly plans with patients
• Monitoring patient compliance with the plan of care & devices
• Coaching patients on healthier behaviors
• Act as first contact for patient motivationg factors and leveraging them for best possible health outcomes
• Proactively managing patients to prevent urgent care or emergency room visits

Required Experience:

• 2 years nursing experience; case management or home health preferred
• Active Compact Licensure
• EMR Experience
• Computer competency
• Experience caring for some or all of following conditions:
• Diabetes
• COPD
• Asthma
• CHF
• A Fib
