Job summary

Sunrise Community of Tennessee, Inc. is seeking a licensed practical nurse to provide medical services to individuals with intellectual and developmental disabilities.

The role requires a valid LPN license and at least 3 years of driving experience, with basic computer skills essential for documentation. You will assist in maintaining health plans, monitor vitals, and collaborate with Direct Support Professionals to ensure individualized care.

Qualifications

• Must possess current and valid Licensed Practical Nurse license.
• Minimum of 3 years of driving experience with endorsements.
• Must have basic computer skills.
• Experience working with individuals with intellectual/developmental disabilities preferred.

Responsibilities

• Assist in providing basic healthcare services per individual plans.
• Monitor blood pressure, weights, and other assigned tasks.
• Maintain accurate and complete documentation per regulations.
• Ensure organization and restocking of supplies and equipment.
• Collaborate with Direct Support Professionals to meet individuals' needs.
• Report changes in medical or behavioral status to supervisor.
