Job summary

Paul House & Health Care Center is seeking compassionate Licensed Practical Nurses (LPNs) to join our nursing team in Chicago, IL. LPNs will provide resident-centered care and collaborate with CNAs, nursing leadership, physicians, and the interdisciplinary team.

Qualifications

• Current and valid LPN license in Illinois.
• Graduate of an accredited practical nursing program.
• Strong clinical, communication, and organizational skills.
• Ability to supervise and delegate to nursing staff.
• Compassionate, dependable and professional care.
• Long-term care or skilled nursing experience preferred.
• Must meet health and background requirements.

Responsibilities

• Provide safe nursing care to residents.
• Administer medications and treatments per orders.
• Monitor residents and report significant changes promptly.
• Complete accurate nursing documentation.
• Assist with the development and implementation of resident care plans.
• Supervise and delegate to CNAs.
• Communicate effectively with residents, families, physicians, and the interdisciplinary team.
• Collaborate with the Director of Nursing and nursing leadership.
• Maintain resident dignity, privacy, safety, and quality of care.
• Follow facility policies, infection-control practices, and applicable regulatory requirements.
• Perform additional nursing duties as assigned.

Our LPNs play an important role in providing quality, resident-centered care while working collaboratively with CNAs, nursing leadership, physicians, and other members of the interdisciplinary team.

Schedule

Available 12-hour shifts include:
