Job summary

Paul House & Health Care Center in Chicago, IL is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our caring nursing team. You will provide safe, resident-centered care and collaborate with CNAs, nursing leadership, physicians, and the interdisciplinary team.

We offer both full-time and part-time positions across AM and PM shifts, with opportunities for training, professional development, and leadership growth within a supportive work environment.

Qualifications

• Current and valid Licensed Practical Nurse (LPN) license in the State of Illinois
• Graduate of an accredited practical nursing program
• Strong clinical, communication, and organizational skills
• Ability to supervise and appropriately delegate to nursing staff
• Compassionate, dependable, and professional approach to resident care
• Long-term care or skilled nursing experience preferred
• Must meet applicable health and background requirements

Responsibilities

• Provide safe and appropriate nursing care to residents
• Administer medications and treatments in accordance with physician orders and facility policies
• Monitor residents and promptly report significant changes in condition
• Complete accurate and timely nursing documentation
• Assist with the development and implementation of resident care plans
• Supervise and appropriately delegate responsibilities to CNAs
• Communicate effectively with residents, families, physicians, and members of the interdisciplinary team
• Collaborate with the Director of Nursing, Assistant Director of Nursing, and other nursing leadership
• Maintain resident dignity, privacy, safety, and quality of care
• Follow facility policies, procedures, infection-control practices, and applicable regulatory requirements
• Perform additional nursing duties as assigned

Education

Licensed Practical Nurse (LPN) license (Illinois)

Graduate of an accredited practical nursing program
