Job summary

Angels Care Home Health is currently seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes. The role includes coordinating with the interdisciplinary team, patients, and referring agencies while ensuring high-quality care and adherence to care plans.

Applicants must hold an active LPN license, a valid driver’s license, and CPR certification, with two years of LPN experience preferred in home health.

Qualifications

• Graduate of an accredited school of practical nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
