Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients’ homes under the supervision of a registered nurse. You will coordinate care with families, interdisciplinary team, and referring agencies to ensure high-quality in-home services.

The ideal candidate is a graduate of an accredited LPN program, holds a current LPN license, has a valid driver's license, and at least two years of home health experience.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under RN supervision in patients’ homes.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
• Ensure safety and quality of care during home visits.
