AlwaysNear Home Care LLC in Atlanta, GA is seeking a compassionate Licensed Practical Nurse (LPN) to join our growing home health team. You will deliver high-quality, patient-centered care in patients' homes, supporting families and coordinating with RNs and physicians.

The role emphasizes wound care, medication administration, vital signs, and developing individualized care plans. Strong communication and reliability are essential as you travel throughout Atlanta to support patients in need.

Qualifications

• Active LPN license in Georgia.
• Current CPR/BLS certification required.
• Prior home health, long-term care, or clinical nursing experience preferred.
• Strong knowledge of medication administration, wound care, and patient assessment.

Responsibilities

• Provide direct patient care including wound care, medication administration, and vital sign monitoring.
• Develop and follow individualized patient care plans in coordination with RNs and physicians.
• Observe and document patient conditions, progress, and any changes in health status.
• Assist patients with activities of daily living (ADLs) and personal hygiene as needed.
• Communicate effectively with patients, families, and the broader care team regarding treatment plans.
• Ensure compliance with all state, federal, and agency regulations and standards of care.
• Provide patient and family education on medications, treatments, and home health management.
