AlwaysNear Home Care LLC is seeking a compassionate PRN LPN to provide in-home nursing care in the Atlanta, GA area. The role emphasizes patient-centered care, wound care, medication administration, and coordination with RNs and physicians to deliver high-quality services at patients' homes.

The ideal candidate holds an active Georgia LPN license, CPR/BLS certification, and has experience in home health or related clinical nursing.

Qualifications

• Active LPN license in Georgia
• Current CPR/BLS certification
• Prior home health, long-term care, or clinical nursing experience preferred
• Strong knowledge of medication administration, wound care, and patient assessment
• Excellent communication, organizational, and time-management skills
• Reliable transportation and valid driver's license for home visits in Atlanta, GA
• Compassionate, patient-focused demeanor; pediatric experience a plus

Responsibilities

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans in coordination with RNs and physicians
• Observe and document patient conditions, progress, and changes in health status
• Assist patients with activities of daily living (ADLs) and personal hygiene as needed
• Communicate with patients, families, and care team regarding treatment plans
• Ensure compliance with state, federal, and agency regulations and standards of care
• Provide patient and family education on medications, treatments, and home health management
