Job summary

Presidential Home Care LLC in Norcross, GA is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health team. You will deliver high-quality, personalized nursing care in patients' homes, collaborate with RNs and physicians, and support families in managing care plans.

The ideal candidate has a GA LPN license, strong clinical skills, excellent communication, and the ability to work independently in a home setting. CPR/BLS preferred; valid driver's license required.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Georgia.
• Prior home health, long-term care, or clinical nursing experience preferred.
• Strong knowledge of nursing procedures, medication management, and patient assessment.
• Excellent communication, organizational, and time-management skills.
• Ability to work independently and exercise sound clinical judgment.
• Valid driver's license and reliable transportation required.
• CPR/BLS certification preferred.

Responsibilities

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans in coordination with supervising RNs and physicians
• Observe and document patient health status, progress, and any changes in condition
• Assist patients with activities of daily living (ADLs) and personal hygiene as needed
• Educate patients and family members on care procedures, medications, and health management
• Communicate effectively with the healthcare team to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with state and agency regulations
