Quadrant, Inc. is seeking an experienced Licensed Practical Nurse to coordinate nurse-led care in our Alexandria clinic.

The ideal candidate holds an active Virginia LPN license, BLS, and at least two years of LPN experience in pediatrics, behavioral health, or family medicine, with familiarity in healthcare insurance processes.

Responsibilities

  • Facilitates daily flow of medical care in clinic
  • Performs triage by phone or in person, and nurse visits for chronic care education and management
  • Collaborates with a multidisciplinary team to deliver high-quality primary care
  • Manages high-risk patient care with complex comorbidities
  • Responds to patient questions and requests for care with the care team
  • Informs patients of lab results and need for follow-up care
  • Administers medications and immunizations
  • Provides patient education
  • Assists with ordering and maintenance of supplies and equipment
  • Obtains prior authorizations for medications and urgent procedures
  • Assists with Virginia vaccine program inventory and orders
  • Helps patients navigate the health care system
  • Participates in clinic quality improvement and risk management
  • Tracks follow-up for procedures and results; coordinates off-site scheduling
  • Fills in for medical assistant duties as needed
  • Prepares internal and external reports as requested
  • May lead project roles to advance department
  • May assist weekends or evenings
  • Able to work under minimal supervision

Licensed Practical Nurse - Nurse Coordinator

Alexandria, VA
Pay From: $33 per hour

MUST
  • Experienced Licensed Practical Nurse
  • Must have an active Licensed Practical Nurse in Virginia
  • Must be BLS certification
  • 2+ years of experience as a Licensed Practical Nurse
  • 2+ years of experience working with specialty in Pediatric, Behavioral Health and/or Family Medicine
  • 2+ years of experience with healthcare insurance
  • Ability to work effectively with a diverse workforce and patient base
  • Ability to work in a fast pace environment
  • Attention to detail and excellent technical skills
DUTIES
  • Facilitates daily flow of medical care in clinic
  • Performs clinical duties, including phone or in-person triage, nurse visits for chronic care education and management, and communication of clinical information
  • Works with multidisciplinary team, to include the medical provider, behavioral health provider, care coordinator, and medical assistant to provide high quality, coordinated, and patient-centered primary care
  • Manages high risk patient care, including patients with multiple co-morbidities or those at high risk for hospitalization or readmissions
  • Responds to patient phone calls, questions, and requests for care, in coordination with medical patient care team
  • Informs patients of lab and diagnostic results and need for follow up care
  • Administers medications and immunizations
  • Provides education to patients
  • Assists with ordering and maintenance of clinical supplies, onsite medications, and equipment
  • Obtains prior authorization for medications and urgent patient procedures
  • Assists with administration of Virginia state vaccine program by performing vaccine inventory and assisting with orders
  • Along with care coordinators, assists patients in efficiently navigating the health care system
  • Participates in clinic quality improvement and risk management activities involving individual patient outreach, population health campaigns, and chronic care management
  • Tracks patients who need follow up GYN procedures and diagnostic tests, coordinates receipt of results, and assist patients with scheduling procedures off site
  • Tracks patients with abnormal cancer screening studies to ensure follow up care is provided
  • Fills in for medical assistant duties as needed
  • Prepare and maintain special internal and external reports as requested by Supervisor and/or Department Head.
  • May be assigned to various project lead roles to advance the development of the department and support ad hoc needs
  • Assists with developing, coordinating, and/or recommending improvements in program area
  • May be asked to assist during weekend or evening
  • Able to work under minimal supervision
  • Perform additional duties and take on other responsibilities as dictated by events and circumstances
