Role Description

We are seeking dedicated and compassionate Licensed Practical Nurses (LPN) to join our Primary Care Management team. The LPN/LVN/ in Primary Care Management plays a vital role in providing high-quality healthcare services to patients and assisting in the coordination and management of their care. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for patient-centered care. Designed as a hybrid position, the vast majority of employee responsibilities will be able to be completed from the home environment.

Qualifications

1. Current and valid LPN/LVN Compact license.

2. Graduation from an accredited LPN/LVN program.

3. CPR certification.

4. Strong clinical skills and knowledge of primary care principles.

5. Excellent interpersonal and communication skills.

6. Ability to work effectively in a team-based healthcare environment.

7. Proficiency in using electronic health records (EHR) and healthcare software.

8. Compassion and dedication to providing patient-centered care.
