Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities.

Qualifications

• Licensed Physical Therapy Assistant in good standing.
• Open to therapists at all experience levels.

Responsibilities

• Apply evidence-based interventions to improve function.
• Provide skilled therapy under supervision of a Physical Therapist.
• Assess progress and communicate adjustments to the supervising PT.
• Counsel patients and families about physical disorders.
• Assist with discharge summaries per regulatory requirements.
• Document patient progress and treatment details.
