Job summary

Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to deliver rehabilitative services under the supervision of a Physical Therapist across our Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona, and Missouri.

The position offers $42 hourly compensation and is open to therapists at all experience levels who are passionate about patient care, evidence-based practice, and helping patients regain function and quality of life.

Qualifications

• Licensed Physical Therapy Assistant credential required.
• This position is open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients under supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation services.
• Assesses and documents patient progress; communicates with supervising Physical Therapist to adjust treatments.
• Provides counseling to patients and families related to physical disorders.
• Input for discharge summaries as required.
