Job summary

Kaiser Permanente is seeking a dedicated Physical Therapist to manage the rehabilitation needs of patients across inpatient and outpatient settings. You will evaluate, plan, administer and coordinate therapy services, while documenting per regulations and contributing to discharge planning.

Strong collaboration with care teams and educating patients and families are essential. The role requires a CA PT license (or eligibility), valid NPI, and BLS.

Qualifications

• Completion of an accredited PT training program with a Bachelors, Masters, or Doctorate in Physical Therapy.
• California PT license required at hire or eligible at hire (PTLA).
• NPI required at hire.
• BLS from American Heart Association required at hire.

Responsibilities

• Evaluates patients and interprets examination findings to determine dysfunction and restore movement and daily activities.
• Establishes plan of care to achieve goals based on age and medical history.
• Administers modalities and exercises as part of patient care.
• Coordinates treatment plan and participates in discharge planning with other health professionals.
• Supervises PT assistants and aides as appropriate.
• Documents services accurately per state and medical group policies.
• Provides patient and family education and participates in rounds/case management as needed.
• Complies with Kaiser Permanente policies and department guidelines.
• Availability for inpatient acute, orthopedic, neuro, and other settings is highly preferred.
