Job summary

Apple Rehab Rocky Hill is a 100-bed skilled nursing center in Rocky Hill, CT, offering a dynamic Physical Therapist role to help patients recover and maximize function. The PT will evaluate patients, develop individualized rehabilitation plans, and coordinate with the care team to ensure integrated therapies for optimal outcomes.

Qualified candidates are graduates of an accredited PT program with a current PT license.

Qualifications

• Graduate of an accredited PT program.
• Current PT license, in good standing with the State licensing board.

• Federal-, State- and GHR-required credentials current and on file.

Responsibilities

• Plans and administers medically prescribed physical therapy treatment for patients to restore function, relieve pain, and prevent disability.
• Reviews physician's referral and patient condition and medical records to determine required therapy.
• Tests and measures patient strength, motor development, sensory perception, functional capacity, and records findings to guide treatment.
• Plans and prepares written treatment program based on evaluation of patient data.
• Instructs and assists patients in performing exercises and activities.
• Administers physical therapy modalities and uses equipment appropriately.
• Contributes to care planning with physicians and other practitioners.
