Job summary

Safe Life Home Health Care, LLC is seeking a qualified Physical Therapist to provide PT services in patients' homes within Morris, IL and surrounding Grundy County. You will supervise assistants, implement plans of care, and coordinate with physicians and the DON.

The role requires IL licensure, at least one year of home health experience, reliable vehicle with insurance, and willingness to travel to patient residences. A compassionate, patient-centered approach is essential.

Qualifications

• Illinois license as Physical Therapist.
• Minimum 1 year of home health care clinical experience.
• Car with valid auto insurance.
• Annual TB test and CPR certification.
• Able to read and interpret patient care instructions.
• Able to manage multiple tasks and priorities.
• Must be willing to travel to patients' residences.

Responsibilities

• Assist physician in evaluating patients’ functional level and set goals.
• Perform initial evaluations within 48 hours of referral and re-evaluations every 30 days.
• Prepare clinical and progress notes and required summaries.
• Revise plan of treatment and communicate changes to physician.
• Advise and consult with patient’s family and other health care staff.
• Coordinate care with agency DON and other staff.
• Observe, record, and report changes in patient status or schedule changes.
• Supervise Physical Therapy Assistant.
• Instruct patient/family in equipment use and home therapy programs.
• Communicate effectively with patients, caregivers, and staff.
• Ensure timely completion of all tasks.
