Job summary

Safe Life Home Health Care, LLC is seeking a Physical Therapist to provide compassionate home health services in Ottawa and surrounding LaSalle County, IL. You will evaluate patients, develop plans of care, and coordinate with physicians and the care team to ensure optimal outcomes.

The role requires Illinois licensure, at least one year of home health experience, and the ability to travel to patients’ residences. A vehicle and insurance are required to perform visits efficiently.

Qualifications

• Must be a licensed Physical Therapist by the state of Illinois.
• Must have at least one year of home health care clinical experience.
• Must have a car with valid insurance and be able to travel to patients' homes.

Responsibilities

• Assists physicians in evaluating patients and establishing treatment plans.
• Performs initial evaluations within 48 hours of referral and ongoing re-evaluations as needed.
• Maintains documentation and communication with care team and patients’ families.
• Supervises PT Assistants and coordinates care with agency DON and staff.
• Educates patients and caregivers on home therapy and equipment usage.
