Job summary

Safe Life Home Health Care, LLC is seeking a Physical Therapist to provide PT services in the Ottawa area and surrounding La Salle County, IL. The role involves evaluating patients, developing and updating plans of care, and supervising PT assistants as part of a coordinated home-health team.

The candidate must be licensed in Illinois, have at least one year of home health experience, own a car with insurance, and hold current CPR/TB certifications.

Qualifications

• Licensed Physical Therapist in Illinois.
• At least 1 year of home health care clinical experience.
• Car with valid insurance coverage.
• TB test and CPR certification current.
• Able to travel to patients' residences.

Responsibilities

• Assist physicians in evaluating patients’ function, establish treatment goals.
• Perform initial evaluations within 48 hours of referral and re-evaluations every 30 days.
• Prepare clinical and progress notes; update care plans with physician as needed.
• Instruct patients and families in home therapy programs and safe equipment use.
• Supervise Physical Therapy Assistant and maintain quality care.
