Job summary

Safe Life Home Health Care is seeking a Licensed Physical Therapist to provide professional PT services in patients’ homes in Dixon, IL. The role includes supervising PT Assistants and ensuring follow-through of the plan of care as directed by agency policy.

You will perform initial evaluations, coordinate with physicians, and communicate with patients and families. Travel to patient residences is required; state license and CPR are mandatory.

Qualifications

• Must be a licensed Physical Therapist in Illinois.
• Minimum of 1 year of Home Health Care clinical experience.
• Valid driver’s license with auto insurance.
• CPR certification required.
• Must be able to travel to patients’ residences.
• Ability to read and interpret patient care instructions.
• Ability to prioritize multiple tasks and manage time effectively.

Responsibilities

• Assist the physician in evaluating patients’ function, establish treatment plans and goals.
• Perform initial evaluations within 48 hours of referral and re-evaluations every 30 days.
• Prepare clinical and progress notes and required summaries.
• Revise plan of treatment and coordinate with physician as needed.
• Advise and consult with patient’s family and other health care staff.
• Coordinate care with DON and other staff.
• Observe and report changes in patient condition and schedule.
