Job summary

Safe Life Home Health Care, LLC seeks a licensed Physical Therapist to provide home health PT services in Rockford and surrounding areas. You will supervise PTAs, evaluate patients within 48 hours, and coordinate care with physicians and the agency team.

The role requires a valid IL license, travel to patients’ homes, and delivering high-quality care across multiple visits and settings. W2/contract options may apply, with varied hours to meet patient needs.

Qualifications

• Must be a licensed Physical Therapist by Illinois.
• Minimum of 1 year home health clinical experience.
• Must have a vehicle with current insurance.
• Annual TB test and CPR certification required.
• Able to read and interpret patient care instructions.
• Able to travel to patients' homes.

Responsibilities

• Assist physician in evaluating patients and set treatment goals.
• Perform initial evaluations within 48 hours of referral; re-evaluate every 30 days.
• Prepare clinical and progress notes and summaries.
• Revise plan of care and coordinate with physician.
• Advise and consult with patient’s family and other staff.
• Coordinate care with DON and staff.
• Observe and report changes in patient condition to agency.
• Supervise the Physical Therapy Assistant.
• Instruct patient/family on equipment use and home therapy.
• Communicate with patients, caregivers, and staff.
• Provide quality care and report concerns to DON/Administrator.
• Ensure timely completion of tasks.
• Check in on PTAs every 4-6 patient visits.
