Job summary

Continuum Healthcare Inc. seeks a licensed Physical Therapist Assistant (PTA) for a full-time role in elder care. You will support plan of care and work with an interdisciplinary team to ensure quality resident rehabilitation.

Responsibilities include assisting in therapy sessions, documenting progress, traveling to other facilities if needed, and contributing to center operations under supervision of a licensed therapist. Indiana license required.

Qualifications

• Graduate with an associate’s degree in physical therapy or occupational therapy.
• Licensed or certified as PTA in the designated state(s).
• Prefer experience in older adult rehabilitation.

Responsibilities

• Provides resident rehabilitation care within established plan of care.
• Collaborates with supervising therapist and communicates resident status.
• Performs necessary documentation and reporting.
• Participates in center processes and services as directed.
• May travel to other facilities to provide treatments.
• Assists with marketing efforts as directed.
