Responsibilities

• Assist the Physical Therapist with treatment plans and data collection during screenings.
• Implement treatment plans including exercises, modalities, and mobility training.
• Monitor residents and report changes to the Physical Therapist.
• Maintain accurate documentation of resident progress and interventions, including EMRs (Net Health/PCC).
• Educate residents, families, and staff on mobility techniques and exercises; conduct remote check-ins with discharged patients.
• Collaborate with the interdisciplinary team under the guidance of the Physical Therapist.
