Job summary

Evolve-Therapy-Services in sunny Florida is seeking an Occupational Therapist to lead in-house skilled nursing facility services. This full-time opportunity offers a flexible schedule and relocation assistance, with potential for permanent placement after six months.

You will plan, organize, develop, and direct OT services, screen residents, develop treatment plans, and coordinate with the interdisciplinary team. Active OT license in the state and ongoing continuing education required.

Responsibilities

• Screen residents to determine best intervention for each patient
• Evaluate residents to inform and develop treatment plans with short- and long-term goals
• Communicate evaluation findings and plans to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules with the team
• Perform therapy per plan and document/bill OT services
• Oversee COTA and ensure documentation and reimbursement practices are followed
