Job summary

FOX Rehabilitation in Grapevine, TX is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in homes and senior living communities. You will conduct evaluations, create plans of care, and provide evidence-based treatment that improves mobility and independence.

Qualifications

• OT license in TX or eligibility to obtain.
• Degree from accredited OT program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one OT to older adults in homes or Senior Living Communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
