Aston Group is seeking a compassionate licensed Certified Occupational Therapist (OT) to join our Skilled Nursing Facility rehabilitation team in Jacksonville, FL. Qualifications

• Current OT license in good standing in Florida.
• Preferred experience in skilled nursing or long-term care.
• Knowledge of Medicare regulations.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy focused on ADLs and functional mobility.
• Monitor resident progress and adjust treatment plans.
• Complete documentation in compliance with guidelines.
• Participate in interdisciplinary care planning and discharge planning.
• Supervise COTAs as applicable.
• Educate residents and families on adaptive techniques.
• Maintain compliance with regulations.
