Job summary

Renewal Rehab is hiring a Licensed Occupational Therapist for a full-time position in Streator, IL. You will assess and treat patients, develop individualized care plans, and collaborate with a care team to achieve optimal outcomes in a skilled nursing/long-term care setting.

Qualifications

• Must be a licensed Occupational Therapist to apply.
• Degree in OT from an AOTA-accredited program.
• NBCOT certification and current state OT license (as required).

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
