Job summary

Renewal Rehab, part of the Enhance Therapies family, is hiring an Occupational Therapist for a full-time role in Streator, IL. You will evaluate and treat patients with functional, cognitive, and ADL impairments and create personalized care plans within a skilled nursing/long-term care setting.

Must hold an OT degree from an AOTA-accredited program, be NBCOT certified where required, and possess an active OT license in the state.

Qualifications

• Degree in Occupational Therapy from an AOTA-accredited program.
• NBCOT OTR certification where required by state.
• Current OT license in the state of practice.
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
