Renewal Rehab in Macomb, IL is seeking a licensed Occupational Therapist for a full-time position in a skilled nursing facility. You will evaluate patients, design care plans, and collaborate with the care team to achieve quality outcomes.

Qualifications

• Must be licensed as an Occupational Therapist to apply.
• OT degree from AOTA-accredited program.
• NBCOT OTR certification (state requirements may apply).
• Current state license as an Occupational Therapist.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are obtained and up to date.
