• Active Vermont LNA certification in good standing.

Responsibilities

• Provide direct patient care including bathing, grooming, dressing, and personal hygiene assistance.
• Monitor and record vital signs, reporting changes in patient condition to nursing staff.
• Assist patients with mobility, transfers, and ambulation using proper body mechanics and safety protocols.
• Support patients with meals, hydration, and nutritional needs.
• Maintain a clean, safe, and comfortable environment for patients and residents.
• Document care activities accurately in accordance with facility policies and compliance standards.
• Communicate effectively with patients, families, and interdisciplinary care teams.
