• Monitor and record patient vital signs, reporting changes in condition to nursing staff.
• Assist patients with mobility, transfers, and ambulation using proper body mechanics and safety protocols.
• Support patients with meals, feeding, and nutritional intake documentation.
• Maintain a clean, safe, and comfortable environment for patients and residents.
• Communicate effectively with RNs, LPNs, and interdisciplinary care team members.
• Provide emotional support and companionship to patients and their families.
